We have developed this form to help collect these critical stories of health status segregation and discrimination and share insights with legislators. We must show that this issue is real and pervasive, and they must put laws in place that protect all of us from unjust medical discrimination. We need all those stories! Help protect not only yourself from future discrimination, but, let’s make sure this never happens again, and we protect our children’s future as well!
